Es importante destacar que la alergia alimentaria es tan sólo una de las numerosas posibles razones para que exista intolerancia alimentaria. La intolerancia alimentaria se puede definir como una condición en la que se producen efectos adversos tras ingerir un alimento en concreto o un ingrediente culinario. La intolerancia alimentaria genuina es distinta de la aversión alimentaria psicológica, en la que la persona odia una comida y cree que el alimento en cuestión le produce una reacción determinada. La alergia alimentaria genuina se produce cuando tiene lugar una reacción inmunológica determinada en el cuerpo como respuesta a la ingestión de un alimento determinado. Las alergias se manifiestan en grupo, de manera que las personas que son alérgicas a determinados alimentos también pueden ser alérgicos a otros factores medioambientales, como por ejemplo, al polvo, al pelo de los animales o al polen. Una respuesta alérgica verdadera implica una reacción anormal o alterada del tejido a un antígeno. Un antígeno puede ser una proteína, una sustancia unida a una proteína, un aditivo añadido a un alimento, o menos comúnmente, un polisacárido. El antígeno se combina con un anticuerpo y produce una respuesta inmunitaria, que acaba en un daño celular y la liberación de histamina. El sistema inmunitario desempeña un papel primordial en nuestros cuerpos al protegernos de la invasión de sustancias perjudiciales. Una alergia se produce cuando el sistema inmunológico no funciona de forma adecuada ante una sustancia no perjudicial, como por ejemplo, una determinada proteína alimentaria. Las intolerancias alimentarias, que no sean por alergia, se pueden dar como consecuencia de diversos factores, entre los que se incluyen: ***- Liberación no alérgica de histamina*** Los síntomas son muy similares a los de una alergia, y pueden ser dolor de cabeza, tumefacción, urticaria, vómitos y diarrea. Una sustancia llamada histamina se libera (también se libera en las reacciones alérgicas auténticas) como respuesta a alimentos como el marisco o las fresas. ***- Deficiencias del metabolismo*** La carencia o la deficiencia de las enzimas responsables de la digestión alimentaria pueden causar muchas clases de intolerancia alimentaria. Por ejemplo, una deficiencia de lactasa, la enzima responsable de la digestión de la leche, causa intolerancia a la leche. La enfermedad celíaca es una intolerancia del intestino a una proteína que se encuentra en el trigo, llamada gluten; no debería considerarse una alergia. Los síntomas de la enfermedad celíaca se controlan siguiendo una dieta sin gluten. Se desconoce exactamente cómo y porqué el gluten daña el intestino, aunque en la actualidad se cree que se trata más bien de una respuesta inmunológica anormal y no de una deficiencia de la enzima. Tampoco se considera una alergia alimentaria en el sentido estricto de la definición. ***- Efectos farmacológicos*** Algunas sustancias alimentarias pueden actuar como fármacos, especialmente si se toman en grandes cantidades. La más conocida de estas sustancias es la cafeína, que se encuentra en el té, el café, el chocolate y en refrescos de cola. Una gran ingesta de cafeína puede causar temblores, migraña y palpitaciones. Entre otras sustancias activas farmacológicas que se encuentran en los alimentos están la histamina, la tiramina, la triptamina y la serotonina, que se pueden consumir en alimentos como el vino tinto, el queso, el extracto de levadura, los aguacates y los plátanos. En las personas sensibles, estos alimentos les pueden producir urticaria, rubor y dolores de cabeza. ***- Intolerancia alimentaria de origen desconocido*** Las reacciones pueden originarse como consecuencia de muchos alimentos y productos alimenticios que todavía no se conocen del todo. Pueden ser o no ser reacciones alérgicas. Los aditivos alimentarios, concretamente la tartracina y el benzoato sódico, pueden provocar urticaria, rinitis y asma. Las levaduras pueden producir una serie de reacciones en algunas personas, sobre todo alteraciones en la piel. Causas comunes de la intolerancia alimentaria Las intolerancias alimentarias más comunes, por orden de frecuencia, son las siguientes: leche, huevos, nueces, pescado/marisco, trigo/harina, chocolate, colorantes artificiales, cerdo/panceta/beicon, pollo, tomate, fruta blanda, queso y levadura. Aunque no todas las intolerancias alimentarias están relacionadas con la carne y los productos derivados de la leche, de la lista de arriba se deduce que las personas vegetarianas, y en concreto las veganas, sufrirán menos intolerancias alimentarias porque ya han eliminado de su dieta algunas de las causas de la intolerancia. Síntomas Entre los síntomas más comunes de la alergia aparecen el asma, los síntomas gastrointestinales (nausea, vómitos y diarrea), eczema, urticaria (ronchas), rinorrea (fuerte obstrucción de la nariz) y angioedema (tumefacción de los vasos sanguíneos). Otros síntomas a largo plazo son la depresión, la ansiedad, la fatiga, la migraña, el insomnio y la hiperactividad en los niños. Tratamiento Como algunas veces resulta realmente difícil distinguir entre una alergia alimentaria genuina y una intolerancia alimentaria, el tratamiento de ambas es a menudo similar. El primer paso consiste en diagnosticar la intolerancia alimentaria. No debe realizarse sin la supervisión de un médico, porque algunas reacciones a la intolerancia alimentaria pueden ser peligrosas. Algunas veces la causa de la intolerancia a un alimento determinado es obvia porque el efecto inmediato se produce al ingerir un alimento en concreto. En este caso, el tratamiento será simplemente evitar ese alimento determinado. Pero en la mayoría de los casos, el alimento sospechoso es más difícil de detectar. En ocasiones el hecho de anotar diariamente los alimentos ingeridos y los síntomas experimentados ayuda a detectar los alimentos o el alimento que producen la intolerancia alimentaria. Otros factores, como pueden ser el tiempo, los ciclos menstruales y relaciones difíciles, pueden afectar los síntomas. Algunas veces, se recomiendan simples dietas de exclusión cuando las anotaciones continuas de un alimento en concreto sugieren que puede ser el causante de la intolerancia alimentaria. Así, por ejemplo, la leche, el huevo o el trigo se pueden excluir de la dieta para observar si se produce una mejora de los síntomas. También se puede recomendar el seguimiento de otras dietas más restrictivas, que sólo incluyen una serie limitada de alimentos que en muy raras ocasiones producen una reacción. Estas dietas generalmente se conocen como dietas de exclusión. El objetivo de una dieta de exclusión es identificar una alergia alimentaria o una intolerancia alimentaria limitando la dieta a una muy pequeña variedad de alimentos, comprobando los síntomas y posteriormente introduciendo de manera muy gradual alimentos de prueba para ver si se produce una reacción. No se debe seguir ninguna dieta de exclusión si no se cuenta con supervisión nutricional.
